Phoenix Performance Academy (PPA)

Liability Waiver & Release Agreement

By registering for and participating in any Phoenix Performance Academy (PPA) program, clinic, training session, league, tournament, camp, evaluation, or event, I acknowledge and agree to the following terms and conditions:

Assumption of Risk

I understand that participation in field hockey and related athletic activities involves inherent risks, including but not limited to falls, collisions, physical contact, equipment-related injuries, overexertion, illness, permanent disability, and

in rare cases, death.

I voluntarily choose to allow myself or my child to participate in PPA activities with full knowledge of these risks and assume all responsibility for any injury, illness, damage, loss, or accident that may occur as a result of participation.

Medical Authorization

I certify that the participant is physically able to participate in athletic activities and has no medical condition that would prevent safe participation.

In the event of an emergency, I authorize Phoenix Performance Academy staff, coaches, trainers, or representatives to obtain emergency medical treatment for the participant if I cannot be reached immediately. I understand that I am financially responsible for any medical treatment provided.

Release of Liability

I, on behalf of myself, my child, my family members, heirs, executors, and assigns, hereby release, waive, discharge, and hold harmless Phoenix Performance Academy (PPA), its directors, coaches, assistant coaches, volunteers, employees, trainers, facility operators, affiliates, and partners from any and all liability, claims, demands, actions, damages, costs, or expenses arising from or related to participation in PPA activities, including claims resulting from negligence to the fullest extent permitted by law.

Insurance

I understand that Phoenix Performance Academy does not provide medical or health insurance coverage for participants. I certify that the participant is covered by adequate personal medical insurance.

Code of Conduct

I understand that athletes and families are expected to demonstrate sportsmanship, respect, and appropriate behavior toward coaches, teammates, opponents, officials, and spectators at all times.

Phoenix Performance Academy reserves the right to remove any participant or spectator from activities for inappropriate conduct, unsafe behavior, bullying, harassment, or repeated violations of club policies, without refund.

Photography & Media Release

I grant Phoenix Performance Academy permission to use photographs, video recordings, live streams, or other media containing the participant’s image or likeness for promotional, marketing, website, and social media purposes without compensation.

Payment & Refund Policy

I understand that registration fees, evaluation fees, tournament fees, and program payments are non-refundable unless otherwise approved in writing by Phoenix Performance Academy.

Acknowledgment

By signing below, I acknowledge that I have carefully read and fully understand this Liability Waiver & Release Agreement. I understand that by agreeing to this waiver, I am giving up certain legal rights on behalf of myself and/or my child.
